Laboratory Results

Horizon abides by U.S. Department of Health and Human Services (HHS) ruling, effective April 7, 2014, which allows patients access to their lab test results within 30 days.  The easiest way to obtain results is to go directly to MyBanner Portal which will provide detailed instructions for viewing your results on-line.  To obtain a printed copy of your results, please complete the "Authorization for Release of Medical Information" and bring it to your nearest Horizon site.

The most valuable resource for understanding the test results is your provider because they understand overall details of your health.
